2/16/2024 EnchantaVaughn Approved QMHPA greeted Chevelle upon arrival to the home. Chevelle appeared to be in a relaxed mood evident by her facial expressions such as frown on the face. Chevelle did not present any symptoms of S/I or H/I at the time of the session. QMHPA visited with Chevelle to provide supportive counseling. Focus for todayâs session was to assist Chevelle in making better decisions. A discussion was held with Chevelle to discuss and teach Chevelle that there are specific consequences for negative choices. Chevelle was taught how she could avoid those consequences by applying problem solving skills. Role playing was done to assist Chevelle in learning various problem solving skills in different scenarios. Chevelle was taught the importance of making positive choices in real life situations and how to choose between correct and incorrect behaviors and how to identify each one by thinking of the possible consequences and outcomes before acting. Chevelleâs rationale regarding her thoughts on consequences and self-accountability were discussed. QMHPA informed Chevelle that if she is in conflict with herself about a particular decision, she should always seek guidance from trusted family to assist her in making appropriate decisions; methods for restoring severed family ties were discussed. Chevelle appeared reluctant at first to engage in concept; she was shallow in her responses and stated contradicting facts to the address concept of a positive outlook to approach obstacles. Chevelle was educated on methods she could utilize to improve her self-esteem and how improved self-esteem would increase her desire to make better decisions. Chevelle was initially reluctant to engage in role playing exercise but responded well to prompting and encouragement to participate. Chevelle agreed how important it was to consider the outcome and consequences both negative and positive before taking action. Chevelle was able to verbalize situations when she was faced with obstacles and tough choices and verbalize why she felt she made the right decision.
